Vacuflush Model #?

Does anyone know the model # of the Vacu-Flush heads used in a 1997 Sea Ray 400 Sundancer? I can't find a model # on it anywhere and trying to get parts.

If you remove the base trim pieces I believe you will find a sticker with the info you need.
