I'll put in a vote for USVI (St.Thomas/St John) unless you are scuba divers. If you like to dive the Bonaire is my pick, if not stay away...there's not much else. I stay at the Ritz Carlton in St Thomas and rent a jeep. Nice ferry ride over to St John's, and explore the island, or split you stay between the islands. There's no BS with immigration and all that stuff.
I'd avoid Jamaica like the plague unless you plan on staying in your "compound", as when you leave the premisis you are targeted for handout, buying stuff, drugs and the like. I've been to ochorios and negril, as well as frequent trips for business to Kingston. Hope I never have to go back.
The Caymans are nice, but expensive, but again, if you dive it's fantastic. I prefer Cayman Brac, but theres nothing to do there but hike, dive and drink.
Most of myother destinations are simply for diving. I think you'll find the "tourism attitude" in the USVI's the best around. Plus you can booze at K-mart for about 1/2 what it is in the states....
