Rudder stuffing box

I have a small leak in the starbort rudder packing. The problem it is right behind the waste tank down in a hole when of course you cant get a pipewrench or a hammer has anyone else had to tighten there rudder stuffing box and that small hole area and what did you use. The boat is in the water and I think you screw down on the bottom nut the tighten down on the top packing nut right?
 
you can use a couple adjustable slip nut wrenches like this...you are correct that you first lower the bottom nut and then tighten down of the top nut...then screw the bottom nut up tight against the top nut while holding the top nut with a wrench to keep it from turning....

the biggest problem for most is gaining access to the rudder assembly....
