QSM 11 Fuel Cooler Bracket Failure

Dont know if this is a consistent failure on QSM's and searched both this forum and boatdiesel.com and did not come up with anything. I have approximately 500 hours on the QSM's in a 480. I have already had the typical coolant reservoir bracket failure and am having a service tech to come out to do the transmission cooler bracket replacement.
Seems that any bracket that is bolted onto these engines are incorrectly designed from a fatigue life standpoint. Knowing the issues, I monitor all the "bolt" on parts to this engine. I found that the fuel cooler bracket was slightly misaligned to the water pump intake. This causes the rubber hose connection to be stressed. My assumption is the weight of the water intake hose on the fuel cooler is causing this. Potentially, the bracket that holds the fuel cooler is being "twisted".
The crack got through nearly 3/4 of the way through along the rearward most connection in one two hour trip. Upon another 1/2 hour trip and the bracket completely broke through.
Wondering if anyone else see's this condition? There does not seem to be a Cummins recall on this bracket, but if this fails, it would be the same result as the gear cooler bracket failing. The potential for the boat filling up with water as there is a direct water intrusion path. Since there is a fuel line attached, this could break and worst case ignite.

There was definitely a recall on the transmission cooler bracket which I have already done. This bracket is on the side of the engine just ahead of the water pumps. Just wondering if this is an isolated incident, which from the lack of responses seems to indicate such.
